Description
This position involves providing primary care in a multi-specialty setting.
The role is contract-based, located in Flatbush, NY, and requires a commitment of 40 hours per week, Monday through Friday.
The Registered Nurse will work onsite, participating in patient assessments, care planning, and medical evaluations.
Requirements
Review and develop patient care plans based on needs and organizational policies.
Support the implementation of organizational programs and procedures.
Collect and adjust data to meet program requirements.
Teach and support patients in self-care management.
Follow up on abnormal lab results and manage medication refill requests.
Coordinate outpatient work-up and treatment plans as needed.
Provide direct care for patients requiring specific assessments.
Qualifications
Completion of nursing training from an accredited school of nursing.
Minimum of one to two years' experience in a medical practice, primary care, med-surg, ER, or occupational health.
Current New York State Registered Nurse license.
