Job description
Role Overview: The RN Care Coordinator Senior (RN Sr) assesses transitional care needs, coordinates care across the continuum, and engages with patients and families during complex hospital admissions. They collaborate with physicians and care teams to develop and implement efficient treatment and discharge plans, ensuring timely progression of care and resource utilization.
Key Responsibilities:
Assessment: Conduct psychosocial and resource assessments, partnering with interdisciplinary teams and payers to establish comprehensive care and discharge plans.
Care Progression: Facilitate communication about patient progress, identify and resolve delays, and ensure medical necessity for treatments.
Disposition Planning: Manage discharge planning, develop individualized post-acute care strategies, and coordinate referrals to ensure smooth transitions.
Documentation: Accurately record assessments, interactions, and care plans, tracking trends to improve outcomes.
Precepting/Mentoring: Support onboarding, mentor new staff, and serve as a resource for interns and less experienced colleagues.
Professional Development: Complete required training, participate in goal-setting, and contribute to organizational success.
Qualifications & Skills:
Associates or Bachelor’s Nursing degree (Bachelor’s preferred)
Minimum 3 years as a staff nurse in an acute care hospital
At least 2 years in hospital or payer-based case management
Valid RN licensure (state-specific or multi-state compact)
Basic Life Support certification
Ideal candidates demonstrate strong clinical expertise, collaborative communication, and leadership in care coordination to improve patient outcomes and discharge efficiency.
