Job description
Take on a rewarding 13-week Travel RN opportunity supporting inpatient discharge planning and care coordination in a Level II acute care setting. This role is ideal for an experienced Case Management RN who can work independently, collaborate with interdisciplinary teams, and manage a diverse inpatient caseload.

Guaranteed Hours: 37.5 hours/week
Requirements
Active NY RN License
BSN required
BLS certification required
PRI (Patient Review Instrument) certification required
SCREEN Training certification required
Minimum 1 year of inpatient Case Management experience required
Prior critical care experience preferred
Knowledge of inpatient discharge planning and making appropriate referrals
Ability to work independently while collaborating effectively with an interdisciplinary team
Strong critical-thinking, organization, communication, and time-management skills
Knowledge of CarePort, MCG, and/or InterQual preferred
