nAs a Registered Nurse Case Manager, you will be the heart of our care model, providing 1:1 holistic support to patients and families in their homes. You’ll partner with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy.

nQualifications
nRegistered Nurse Case Manager Requirements & Key Responsibilities:
nnnAssociates, Bachelors, or Masters of an accredited nursing program (RN)
n
nn1+ years of recent acute care experience required; hospice/home health experience preferred
n
nnComplete initial and ongoing comprehensive assessments to determine hospice needs
n
nnDevelop individualized care plans with the patient and family, updating as needs evolve
n
nnAdminister medications and treatments as prescribed
n
nnEducate patients and caregivers on care techniques, medications, diet, and symptom management
n
nnRotate on-call duties and ensure seamless care transitions
n
nnMust have reliable transportation, valid driver’s license, and proof of auto insurance
n
nnAbility to pass DHS background study
n

nnnSchedule: 8:00am to 5:00pm, Full-time position with on-call expectations
