nAs a Registered Nurse Case Manager, you will serve as the cornerstone of patient care, delivering individualized, holistic support to patients in their homes. You’ll collaborate with a care team to develop a personalized plan of care, lead meaningful end of life conversations, and ensure exceptional symptom management with empathy and clinical excellence.

nQualifications
nRegistered Nurse Case Manager Requirements & Responsibilities:
nnnAssociates, Bachelors, or Masters of an accredited nursing program (RN)
n
nn1+ years of recent acute care experience required; hospice/home health experience preferred
n
nnComplete initial and ongoing comprehensive assessments to determine hospice needs
n
nnDevelop individualized care plans with the patient and family, updating as needs evolve
n
nnAdminister medications and treatments as prescribed
n
nnEducate patients and caregivers on care techniques, medications, diet, and symptom management
n
nnRotate on-call duties and ensure seamless care transitions
n
nnMust have reliable transportation, valid driver’s license, and proof of auto insurance
n

nnnSchedule: 8:00am to 5:00pm, Full-time position with on-call expectations
