n nPhoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.
nOur ideal candidate should possess a high level of energy and motivation, coupled with strong managerial, organizational, and interpersonal skills. They must be capable of providing precise and effective guidance and support, even in demanding situations, in order to enhance learning opportunities and foster professional growth.

nResponsibilities
nnnProvide education to patients/family members regarding the process, plan of care, and offers guidance with healthcare decisions.
n
nnConsult with the attending physician and Hospice Medical Director concerning changes in patients condition requiring order changes and/or changes to plan of care.
n
nnCoordinate hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
n
nnConducts initial as well as on-going comprehensive assessments.
n
nnPerform evaluations for hospice care along with informational visits.
n
n nRequirements
nnnMissouri or Compact RN License
n
nnValid driver’s license and auto liability insurance.
n
