You’ll partner with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy.If you're a compassionate Registered Nurse who values meaningful patient connections and autonomy in your practice, we’d love to meet you.Video Link: Why work for St. Croix Hospice?Registered Nurse Case Manager Requirements & Responsibilities:Associates, Bachelors, or Masters of an accredited nursing program (RN)1+ years of recent acute care experience required; hospice/home health experience preferredComplete initial and ongoing comprehensive assessments to determine hospice needsDevelop individualized care plans with the patient and family, updating as needs evolveAdminister medications and treatments as prescribedEducate patients and caregivers on care techniques, medications, diet, and symptom managementRotate on-call duties and ensure seamless care transitionsMust have reliable transportation, valid driver’s license, and proof of auto insuranceBenefits & Schedule:Schedule: 8:00am to 5:00pm, Full-time position with on-call expectationsHealth, dental, vision, and life insuranceCompany-paid short/long-term disability and basic life insurance401(k) with up to 4% company matchTuition reimbursement and professional development opportunitiesPaid time off and paid holidays
