Job description

Home Instead provides personalized, non-medical in-home care that helps older adults remain safe, comfortable, and independent in their own homes. Our Caregivers assist with companionship, personal care, meal preparation, transportation, medication reminders, and other daily activities.

Home Instead is seeking an experienced, relationship-driven Registered Nurse (RN) to serve as a Client Care Coordinator for families throughout the Charleston area. This highly client-facing, field-based position combines nursing knowledge, care coordination, relationship management, and consultative guidance.

The RN Client Care Coordinator builds trust with older adults and their families, helps them navigate important care decisions, develops appropriate non-medical care plans, and maintains strong client relationships throughout their time with Home Instead.

This is a field position and not primarily a desk-based position or a traditional bedside nursing role. A significant portion of the position is spent conducting initial in-home care consultations and assessments, completing Quality Assurance visits, and following up with clients and families throughout Mount Pleasant, downtown Charleston, West Ashley, James Island, Kiawah Island, Seabrook Island, and surrounding communities.

A company vehicle is provided for approved business-related travel.

What You’ll Do

Conduct in-home care consultations and assessments with prospective clients and their families to understand their needs and recommend appropriate non-medical home-care services.

Develop individualized non-medical care plans based on the client’s needs, routines, preferences, and goals.

Educate and guide clients and families as they make important decisions about care for an aging loved one.

Build trust with families and confidently guide qualified prospective clients through the decision to begin services.

Maintain a high level of conversion from qualified care consultations to new clients.

Complete service agreements, care plans, required documentation, and timely follow-up for new clients.

Coordinate with Scheduling and Client Care to support timely and successful starts of care.

Conduct ongoing Quality Assurance visits with existing clients and families.

Recognize opportunities where additional care may improve a client’s safety, well-being, independence, or quality of life.

Proactively address client concerns and collaborate with the internal team to resolve issues and strengthen client satisfaction and retention.

Maintain accurate and timely documentation in company systems.

Participate in the shared weekend on-call rotation, generally serving as backup approximately every fifth weekend for the existing on-call team.

Represent Home Instead professionally throughout the Charleston community.

n Who We’re Looking For

We are looking for a Registered Nurse who combines nursing knowledge and sound judgment with exceptional relationship-building skills and the confidence to ask for the business.

The ideal candidate has experience in home care, home health, hospice, senior living, case management, healthcare admissions, discharge planning, or another client-facing healthcare environment.

You may be a great fit if you:

Hold an active, unrestricted Registered Nurse license in South Carolina or are eligible to obtain South Carolina licensure.

Have one or more years of nursing, care coordination, healthcare admissions, relationship management, business development, or other relevant client-facing experience.

Have experience working with older adults and families navigating complex, stressful, or emotional care decisions..

Can explain care recommendations clearly and compassionately to clients and families.

Have demonstrated success converting qualified inquiries, consultations, or admissions into clients or customers.

Are comfortable discussing pricing and confidently helping families make decisions.

Are comfortable working independently and spending a significant amount of time in the field.

Are organized and capable of managing documentation and multiple client priorities.

Have a valid driver’s license, an acceptable driving record, and the ability to travel throughout the Charleston service area.

n What Success Looks Like

Success in this position means using nursing knowledge, relationship-building skills, and strong follow-through to take ownership of the client experience from the initial care consultation through the successful start and ongoing delivery of services.

Key measures of success include:

Thorough, accurate, and timely in-home assessments and individualized care plans.

Strong conversion of qualified care consultations into new clients, with a target of 80% or greater.

Successful and timely starts of care.

Strong 30- and 60-day client retention.

Consistent completion of Quality Assurance visits and required follow-up.

Early identification and appropriate communication of changes in client condition or care needs.

Accurate and timely documentation and communication.

n Compensation and Benefits

Salary: Compensation is commensurate with experience and qualifications.

Eligibility for a monthly team performance bonus based on established company goals.

Company vehicle provided for approved business-related travel.

Health benefits and 401(k) available to eligible full-time employees.

Paid time off.

Training and ongoing professional development.

n Schedule, Travel, and On-Call Rotation

This is a full-time, field-based position requiring regular travel throughout the Charleston service area, including Mount Pleasant, downtown Charleston, West Ashley, James Island, Kiawah Island, Seabrook Island, and surrounding communities.

Some scheduling flexibility is required because care consultations and client needs do not always occur during traditional office hours.

This position participates in the company’s shared weekend on-call rotation, generally serving as backup approximately every fifth weekend for the established on-call team. This position is not responsible for serving as the company’s primary on-call manager.
