Job description
Director of Care Coordination
Schedule: Day Shift, Monday - Friday
Assignment Details: Temporary position
Requirements:
BSN required; master's degree in nursing, Health Administration, or Business Administration preferred
Case Management certification (e.g., CCM or ACM) preferred
Minimum of five (5) years of experience in an acute healthcare setting required
Minimum five (5) years of progressively responsible leadership/administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function required
Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment
BLS/BCLS certification required
Responsibilities:
Lead interdisciplinary clinical and operational programs and staff
Oversee performance improvement and support regulatory or accreditation readiness
Ensure effective care coordination and efficient patient progression through the healthcare continuum
Develop and implement care coordination strategies to improve patient outcomes
Coordinate with healthcare providers to facilitate patient transitions of care
Promote a collaborative work environment and foster staff development
