nRN Case Manager- Home Health/Home Care

nEmployment Type: Part-Time/ Per-Diem (Flexible Scheduling Available)
nCompensation: Competitive Pay, PRN: $125 SOC Visit / $75 Reassessment Visit + Milegae
nLocations:
nWhat You Will Do:
nnConduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
nPerform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
nProvide hands-on skilled nursing care , including wound care, medication management, injections, and chronic disease management
nDeliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
nCollaborate with physicians and interdisciplinary team members to ensure coordinated, high-quality care
nMaintain accurate and timely clinical documentation in compliance with state regulations and agency standards
nSupervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care
n nWhat You Will Have:
nnActive Registered Nurse (RN) license in the state of practice
nExperience in home health, skilled nursing, or a related healthcare setting preferred
nStrong clinical assessment and critical thinking skills
nKnowledge of home health regulations and documentation standards
nExcellent communication and interpersonal skills
nAbility to work independently and manage time effectively
