Job description
We are seeking a Registered Nurse (RN) Home Health Case Manager to provide comprehensive patient care in patients' homes.
Responsibilities include: managing caseloads, supervising LPN visits, conducting evaluations, initiating care, and developing personalized care plans based on assessments and physician orders.
Collaborate with healthcare professionals in Physical Therapy, Speech Therapy, and Occupational Therapy.
Educate patients on health conditions such as CHF, COPD, Diabetes, Wound Care, and Medication Management.
Communicate effectively with physicians and insurance providers to coordinate services.
Utilize AI-supported charting tools to enhance efficiency and work-life balance.
Qualifications: Valid Kansas or Compact RN license, a driver’s license, vehicle with insurance, and preferred experience in Home Health or Critical Care.
Skills: Strong physical assessment, diagnostic interpretation, and communication skills.
