Job description
We are seeking a Registered Nurse (RN) Home Health Case Manager to provide patient-centered care in clients' homes.
Responsibilities include: managing caseloads, supervising LPN visits, conducting assessments, developing personalized care plans, and collaborating with physicians and therapy teams.
Qualifications: Kansas or Compact RN licensure, valid driver’s license, vehicle with insurance, and experience in home health or critical care preferred.
Skills required: strong physical assessment, diagnostic interpretation, effective communication, and ability to educate patients on health conditions such as CHF, COPD, Diabetes, and Wound Care.
Work environment: delivering hands-on care, coordinating services, and utilizing AI-powered charting tools to streamline documentation, promoting work-life balance.
