Job description
We are seeking a Registered Nurse (RN) Home Health Case Manager to oversee patient care in a home health setting.
Responsibilities: Manage a caseload of home health patients, supervise LPN visits, coordinate with therapy teams, conduct assessments, and develop personalized care plans for conditions like CHF, COPD, diabetes, wounds, and medication management.
Key duties include: Building patient care plans, supporting interdisciplinary teams, and ensuring quality care delivery.
Qualifications & Skills
Missouri or Compact RN license
Valid driver’s license
Strong organizational, clinical, and communication skills
