WSi Healthcare is seeking an experienced Registered Nurse (RN) to support a home health team in the Pueblo area. The primary need is for a full-time RN, but the team is also open to considering part-time nurses based on availability.
/n
This is a strong opportunity for an RN with home health experience who is comfortable working independently in the field and completing Start of Care visits, recertifications, and clinical documentation.
Responsibilities/n

- /n
Complete Start of Care (SOC) visits and recertifications
/n
- /n
Perform patient assessments and provide skilled nursing care in the home
/n
- /n
Review and complete required clinical documentation
/n
- /n
Assist with chart review and ongoing care coordination
/n
- /n
Communicate with the clinical team regarding patient needs and changes in condition
/n
- /n
Maintain accurate and timely documentation
/n
