\ NWSi Healthcare is seekingan experienced Registered Nurse (RN) to support a home health team in the Pueblo area. The primary need is for a full-time RN, but the team is also open to considering part-time nurses based on availability.
\ NThis is a strong opportunity for an RN with home health experience who is comfortable working independently in the field and completing Start of Care visits, recertifications, and clinical documentation.
Responsibilities\ N Complete Start of Care (SOC) visits and recertifications
\ N Perform patient assessments and provide skilled nursing care in the home
\ N Review and complete required clinical documentation
\ N Assist with chartreview and ongoing care coordination
\ N Communicate with the clinical team regarding patient needs and changes in condition
\ N Maintain accurateand timely documentation
