nAt Phoenix Hospice, our Admissions RNs are the first calm presence when hospice care begins. You’ll guide patients and families from curative care into comfort-focused care with compassion, clinical excellence, and dignity. This role blends strong clinical judgment with deep human connection—addressing not only physical needs, but also emotional, spiritual, and psychosocial support.
nYou’ll collaborate closely with intake coordinators, hospice consultants, medical directors, facility staff, and physicians to ensure every admission is smooth, thoughtful, and centered on what matters most.
nRequirements
nnnHospice experience required
n
nnActive Missouri or Compact RN license
n
nnValid driver’s license
n
nnStrong assessment, organization, and communication skills
n
nnAbility to work independently and as part of a team
n
n nWhat You’ll Do
nnnComplete hospice admissions and skilled nursing visits
n
nnProvide compassionate care and support to patients and families
n
nnEducate patients and families on conditions such as CHF, COPD, and diabetes
n
nnProvide wound care and medication management
n
nnDocument patient conditions and communicate changes to physicians
n
nnCollaborate with the interdisciplinary hospice team
n
