Coverage Territory: Bloomington, Pontiac, Streator, Seneca, Ottawa, LaSalle Peru, Dwight, Marseilles, and surrounding areas.
Schedule: 8 PM-8 AM, alternating 4-day and 3-day weeks including weekends in rotation
Salary Range:
$80,000 - $90,000 a year

What You'll Do
As a Transitions Hospice Night Nurse, you'll serve as a critical support member of the interdisciplinary care team during overnight hours — assessing, treating, and communicating patient needs with clinical confidence and genuine compassion. Your responsibilities include:
* Delivering professional nursing care across the full nursing process within your scope of practice
* Conducting thorough patient and family assessments and contributing to individualized care plans
* Completing all visit types as needed to meet overnight patient and family needs
* Administering medications and treatments per physician orders
* Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment
* Recording accurate, timely clinical notes including pain and symptom management outcomes
* Communicating changes in patient condition to physicians and obtaining orders as required
* Coordinating with community health partners to support continuity of care
* Ensuring equipment and necessary services are in place for patients
* Maintaining and growing your clinical skills through continuing education and in-service participation

What You Bring
* Active RN license in the state of employment
* Graduate of an accredited school of Nursing.
* Clinical experience in hospice, home health, or a related setting preferred
* Comfort and composure working independently during overnight hours
* Strong assessment skills and sound clinical judgment in high-pressure moments
* A calm, reassuring presence for patients and families during nighttime distress
* Excellent documentation habits and communication skills
* Valid driver's license and willingness to travel locally
