Job DescriptionCoverage Territory: Bloomington, Pontiac, Streator, Seneca, Ottawa, LaSalle Peru, Dwight, Marseilles, and surrounding areas.
\ NSchedule: 8 PM–8 AM, alternating 4-day and 3-day weeks including weekends in rotation
\ NSalary Range: $80,000 - $90,000 a year

What You'll Do
\ NAs a Transitions Hospice Night Nurse, you'll serve as a critical support member of the interdisciplinary care team during overnight hours — assessing, treating, and communicating patient needs with clinical confidence and genuine compassion. Your responsibilities include:
\ N Delivering professional nursing care across the full nursing process within your scope of practice
\ N Conducting thorough patient and family assessments and contributing to individualized care plans
\ N Completing all visit types as needed to meet overnight patient and family needs
\ N Administering medications and treatments per physician orders
\ N Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment
\ N Recording accurate, timely clinical notes including pain and symptom management outcomes
\ N Communicating changes in patient condition to physicians and obtaining orders as required
\ N Coordinating with community health partners to support continuity of care
\ N Ensuring equipment and necessary services are in place for patients
\ N Maintaining and growingyour clinical skills through continuing education and in-service participation

\ N Active RN licensein the state of employment
\ N Graduate of an accredited school of Nursing.
\ N Clinical experience in hospice, home health, or a related setting preferred
\ N Comfort and composure working independently during overnight hours
\ N Strong assessment skills and sound clinical judgment in high-pressure moments
\ N A calm, reassuring presence for patients and families during nighttime distress
\ N Excellent documentationhabits and communication skills
\ N Valid driver's license and willingness to travel locally
