What You'll Do
Provide routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing settings.
Perform comprehensive nursing assessments, symptom management, medication education, and comfort-focused interventions.
Support patients and families through compassionate communication and ongoing education.
Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
Complete accurate and timely EMR documentation while maintaining high clinical standards.
What You'll Bring
Active Minnesota Registered Nurse (RN) license.
Hospice experience preferred, or strong experience in oncology, palliative care, home health, ICU, or end-of-life care.
Excellent clinical assessment and communication skills.
Ability to work independently in a field-based role.
Reliable transportation and a valid driver's license.
