Job DescriptionCoverage Territory: Buffalo Grove, Northbrook, Des Plaines, Niles, Elk Grove (Cook County/Lake County) and surrounding areas
\ NSchedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation (on-call and rotating holiday requirements)
\ NPay Range: $85,000 - $95,000 a year

\ N What You'll Do
\ NAs an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:
\ N Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
\ N Building and adjusting personalized care plans in collaboration with physicians and care teams
\ N Administering medications and treatments per physician orders
\ N Keeping clinical documentation accurate, timely, and meaningful
\ N Serving as the communication bridge between families, physicians, and community health partners
\ N Educating patients and caregivers so they feel confident and supported
\ N Participating in on-call rotation per team policy

\ N Active RN licensein the state of employment
\ N Graduate of an accredited school of Nursing.
\ N One (1) + year Hospice,home health or related organization experience preferred.
\ N Compassion for patientsand families navigating end-of-life care
\ N Strong clinical judgment and the ability to work independently in the field
\ N Excellent communicationskills across care teams and families
\ N
