nJob Description:

nnnActs as a liaison across the multidisciplinary team to ensure proper orders, medical records, consents, and other required documentation are received.

n

nnCoordinates timely care with physicians and supportive services departments.

n

nnMaintains professional relationships with customers, patients, and team members.

n

nnProvides direct patient care coordination within the Community Care and Transitional Care Programs.

n

nnOther duties as assigned.

n

nnEducates patients, families, caregivers, and community providers to safely perform care and achieve health goals.

n

nnEvaluates the effectiveness of care plans and monitors patient and family responses to achieve desired outcomes.

n

nnIdentifies performance improvement initiatives and assists in designing or implementing programs to address needed changes.

n

nnParticipates in community education and outreach programs to promote health and wellness.

n

nnCollaborates with Imaging Coordinator, RN Navigator, and referring providers to navigate patients through treatment planning, medical management, and surveillance.

n

nnServes as an educational resource for patients and families on treatment planning and medical management.

n

nnFacilitates communication between healthcare team, patients, other departments, and outside entities by answering phone calls, responding to emails, and managing messages in the EMR and patient portal.

n

n nKnowledge, Skills, and Abilities:

n• Ability to provide age-appropriate cancer education. (Required)

n• Strong organizational, communication, and customer service skills. (Required)

n• Knowledge of and ability to utilize in-house and external oncology resources. (Required)

n• Self-motivated with ability to work independently. (Required)

n• Proficient in Microsoft Office, particularly Outlook, Excel, and Teams (Preferred)

n• Proficient with electronic medical record system such as Epic (Preferred)

n• Bilingual (English/Spanish) (Preferred)

nEducation:

n• Bachelor's (Preferred)

nField of Study:

n• Graduate of an accredited school of nursing

n• in nursing

nWork Experience:

n• 1+ professional nursing experience (Required)

n• 1+ professional nursing experience in oncology (Preferred)

nLicenses and Certifications:

n• Registered Nurse (RN) (Required)

n• Basic Life Support - CPR Cert (BLS) (Required)

n• Oncology Certified Nurse (OCN) (Preferred)

n$68,806.40 - $120,723.20
