Physician Orders & Pharmacy Coordination:

Transcribe physicians' orders accurately into the Electronic Health Record (EHR) system, updating Medication Administration Records (MARs) and Treatment Administration Records (TARs).

Develop, evaluate, and update individualized resident care plans to address physical, nutritional, and emotional health goals.

Lead 2nd shift huddles and collaborate closely with physical, occupational, and speech therapists, social workers, dietitians, and attending physicians to ensure holistic care delivery and effective discharge planning.

Shift Leadership & CNA Delegation:

Direct and supervise Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs) on 2nd shift, delegating direct-care duties, monitoring care delivery, and ensuring prompt responses to call lights.

Serve as the clinical escalation point during afternoon and evening shifts, managing emergent clinical situations calmly and coordinating emergency transport when required.

Documentation & Regulatory Compliance:

Maintain meticulous, real-time clinical documentation in the Electronic Health Record (EHR) system, charting status changes, family notifications, physician visits, and treatment outcomes.

Rigorously maintain compliance with HIPAA, state department of health regulations, OSHA standards, and facility infection control policies.
