Job description
Munson Healthcare is seeking a registered nurse to manage a caseload in home health from admission through discharge, using assessment, planning, coordination, and evaluation.
You will educate patients and families, coordinate care with an interdisciplinary team, supervise LPNs and paraprofessionals, and maintain accurate documentation in the EMR. Michigan nursing license required and valid driver's license for patient visits.
