nEssential Job Functions Overview
nCollaborates, coaches, and aids individuals with mental illnesses in a residential (community residence, apartment, SRO, enriched crisis, and transitional housing) setting. Primarily responsible for teaching/ supporting the skills necessary for optimal success living independently. Based upon individual needs and preferences, utilizes recovery-focused, strengths based, person-centered practices to assist individuals in gaining (regaining) skills lost as a result of mental illness. Individual will strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff, and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences.
nnnAssist residents in developing and maintaining a structured daily schedule according to individualized goals and preferences using trauma informed practices.

nnUtilize motivational interviewing (MI) techniques to provide individual support, training, and assistance in working towards achieving one’s personal goals.
n
nnEncourage and participate in social, recreational, educational, and cultural activities.
n
nnProvide resident transportation via Agency vehicle as needed.
n
nnAssist in general program housekeeping to include provision of hands-on assistance to teach the skills needed to maintain personal living spaces.
n
nnDevelop person-centered service plans for the individuals on your caseload.
n
nnAssist residents in monitoring progress on stated goals and provide updates regularly.
n
nnDocument interactions with residents as it relates to their service plan goals.
n
n nExperience, Education, & Physical Qualifications
nHigh School Diploma with two (2) years of relevant experience or an associate degree in human services or related field or bachelor’s degree (preferred). Valid NYS Driver’s License (as driving is an essential function of the position). May be required to lift (minimum of 35 lbs.), push, pull, or carry heavy objects.
