Physical Therapist
Job Summary
We are seeking a dedicated Physical Therapist with experience in home care and home health to provide quality patient care. The role involves a variety of responsibilities including evaluations, start of care (SOC), recertifications (ROC), and daily visits to patients in their homes.
Key Responsibilities
Conduct patient evaluations and assessments.
Provide start of care and recertification visits.
Deliver therapy services to patients with diagnoses such as Congestive Heart Failure (CHF), orthopedic issues, and pneumonia.
Maintain accurate and timely documentation of patient care.
Collaborate with the clinical manager or educator for support while in the field.
Required Qualifications Current Physical Therapist license
Basic Life Support (BLS) certification
Experience in home health and home care settings
Work Schedule
The position involves working five 8-hour shifts, with expectations of weekend rotations and participation in holiday rotations.
Orientation and Training
New hires will receive two weeks of orientation, which will include shadowing and visits with a work coach.
Additional Information Scrubs or business casual attire is acceptable.
Company-provided phone and tablet for work purposes.
Location
