Job description

PRN Clinical Rehab Liaison

We are seeking an autonomous, business-minded clinical professional to join our team as a PRN Clinical Rehab Liaison within the HCA Healthcare Gulf Coast Division. Operating as a vital clinical consultant and market specialist across our expansive regional network, you will perform comprehensive pre-admission screenings, evaluate patients for medical necessity under inpatient rehabilitation criteria, and build strong referral partnerships with physicians and case managers. This pivotal role is open to experienced Registered Nurses (RN), Licensed Physical Therapists (LPT), Registered Occupational Therapists (ROT), or Speech-Language Pathologists (SLP) who possess sharp diagnostic acumen and thrive in a consultative, outcome-driven environment.

Rehab Liaison is a healthcare professional who serves as a bridge between healthcare facilities, referring sources, and potential patients, focusing on increasing patient referrals and admissions to rehabilitation programs by building relationships, educating the community on services, and ensuring a smooth admissions process. They act as a clinical marketer, evaluating potential patients, streamlining the referral-to-admission process, and overcoming barriers to admission while also promoting the facility's programs and outcomes.

What You Will Do In This Role:

You will act as an inpatient rehabilitation expert between internal and external referral sources, and inpatient rehabilitation operations and establish relationships with case managers, physicians, payors, and other referral sources.

You will assist and coordinate the in-take and pre-admission screening process, and accurately document evidence of rehab admission criteria on pre-screen evaluation.

You will function as primary contact for case management and referring physicians, ensuring referral sources receive appropriate and timely follow-up communications regarding their patients and hospital programs.

You will assist in developing the annual strategic plan for the Rehab Unit/Hospital, to include visible community outreach programming and raising community awareness of services available.

What Qualifications You Will Need:

Graduate of an accredited school in clinical specialty is required.

Licensed professional in Texas in a clinical health care specialty (i.e. Registered Nurse, Licensed Physical Therapist, Registered Occupational Therapist, or Speech Language Pathologist/Therapist-SLP) is required.

Current BLS certification is required.

Minimum 2 years of recent clinical care experience.

HCA Houston Healthcare offers a total rewards package that supports the health, life, career and retirement of our colleagues. The available plans and programs include some or all of the following:
