nPosition Summary:
nThe Rehab Manager oversees rehabilitation services within a home health care setting, ensuring high-quality patient care, compliance with regulatory standards, and efficient team operations. This role involves managing therapists (PT, OT, ST), coordinating care plans, and driving clinical excellence.
nKey Responsibilities:
nLeadership & Management
nSupervise and support rehabilitation staff (Physical, Occupational, and Speech Therapists).
nConduct performance evaluations and provide ongoing training.
nManage scheduling and workload distribution for rehab team.
nClinical Oversight
nEnsure therapy services meet patient needs and physician orders.
nReview and approve care plans and documentation for accuracy and compliance.
nMonitor outcomes and implement quality improvement initiatives.
nCompliance & Documentation
nMaintain adherence to state, federal, and agency regulations.
nAudit therapy notes and ensure timely submission of documentation.
nAddress deficiencies promptly to avoid compliance risks.
nPatient Care Coordination
nCollaborate with nursing, physicians, and other disciplines for holistic care.
nResolve patient or family concerns related to rehab services.
nPromote evidence-based practices for optimal recovery.
nOperational & Financial Management
nMonitor productivity and visit rates to meet budget goals.
nAssist in cost control and resource allocation.
nReport metrics to senior leadership.
nQualifications:
nLicensed Physical Therapist, Occupational Therapist, or Speech Therapist (required).
nMinimum 3–5 years of clinical experience in home health or rehab setting.
nPrior supervisory or management experience preferred.
nStrong knowledge of Medicare, OASIS, and home health regulations.
nExcellent communication, leadership, and organizational skills.
nExperience with Kinnser EMR is plus.
