Physical Therapist Job Requirements Bachelor's degree required
Current Physical Therapist license/certification
At least 1 year of Physical Therapy experience preferred/required
Prior home health experience strongly preferred
Strong communication, organization, and time management skills
Proficiency with HCHB preferred
Ability to provide patient-centered care in a home health environment
Physical Therapy Responsibilities Assess and evaluate patient care needs related to mobility, functional status, endurance, stamina, pain, and other medical conditions
Develop individualized treatment plans based on patient needs and clinical findings
Perform therapeutic interventions that support improved mobility, independence, and overall well-being
Monitor patient progress and adjust care plans as needed
Collaborate effectively with the interdisciplinary care team to support positive patient outcomes
Document visits and care plans accurately within the electronic medical record system
Provide compassionate, high-quality care to patients in the home setting
