Physical Therapist Assistant (PTA) - Home Health Travel Job in Columbia, TN

Charting System: Cerner
Weekend Coverage: Occasional
On-Call: Not required
Job Requirements Recent or current home health therapy experience required
At least 6 months of home health experience required
Home health travel experience required
Active Tennessee PTA license or compact license in hand
Ability to work independently in patient homes
Comfortable documenting in Cerner
Strong communication, organization, and time-management skills
Reliable transportation for home visits throughout the Columbia, TN area
Responsibilities Assist Physical Therapists in delivering skilled home health physical therapy services
Carry out treatment plans designed to improve patient mobility, strength, balance, and function
Provide patient education to support safe recovery and progress at home
Document visits accurately and efficiently in the electronic charting system
Collaborate with the care team to support high-quality patient outcomes
Adapt to a patient census of approximately 6-10 visits
Maintain a professional, compassionate approach in every home setting
