Travel Physical Therapist (PT) - Home Health | Charlotte, NC 28269

Job Requirements Active North Carolina Physical Therapist license
Doctoral degree in Physical Therapy or equivalent required
Minimum of 1 year of Physical Therapy experience required
Home health experience strongly preferred
HCHB experience is a plus
Strong clinical judgment, communication skills, and time management
Ability to work independently while delivering patient-centered care
Responsibilities Provide home health physical therapy services to patients in the community
Perform evaluations, develop treatment plans, and adjust care as needed
Support patients with mobility, strength, balance, and functional recovery goals
Document patient progress accurately and in a timely manner
Coordinate with interdisciplinary team members to promote quality outcomes
Travel within assigned coverage areas to deliver visits efficiently
Maintain a professional, patient-focused approach in all home visits
