Travel Physical Therapist Job - Home Health in Wallingford, CT

Coverage Area: Milford, Fairfield, and surrounding towns
Start Date: 10/12/2026
Duration: 14 weeks
Shift: Monday through Friday, approximately 8:30 AM to 4:30 PM
Shift Duration: 8-hour shifts
Hours per Week: 40
Weekly Estimated Pay: $2,030 - $2,190
Openings: 2
Productivity Expectation: 30 points
Weekend/On-Call Requirement: Not required
Mileage Reimbursement: Yes, between patient visits

Job Requirements Active Physical Therapist license in the state of Connecticut
Doctoral degree required
At least 1 year of physical therapy experience preferred, ideally in home health , rehab, or a similar patient-care setting
Strong clinical judgment and the ability to work independently in the field
Excellent communication, time management, and documentation skills
Ability to travel between patient visits within the assigned coverage area
Professional, patient-centered approach with a commitment to quality care
Responsibilities Provide home health physical therapy visits to patients across the assigned territory
Perform assessments and develop individualized treatment plans based on patient needs
Support patients in improving strength, balance, mobility, and functional independence
Deliver timely, accurate documentation for each visit
Collaborate with the interdisciplinary care team to coordinate patient care
Educate patients and caregivers on exercise programs, safety, and recovery goals
Manage a productive daily schedule while maintaining high standards of care
