Job description
Sentara Norfolk General Hospital is seeking a full-time Advanced Practice Provider to join the Case Management Pharmacy Program in Virginia. The role allows work from home or on-site at the hospital, with a Mon–Fri schedule and no weekends or holidays.
The APP will perform health risk assessments, rewrite prescriptions, and manage about 150 patients per month, primarily for Medicaid. Prior Family Medicine APP experience is required; rheumatology or infectious disease experience is a plus.
