Job description
Brooks Rehabilitation is seeking an APRN to provide primary care services remotely, focusing on patients at higher risk for rehospitalization. Responsibilities include medication management, comprehensive assessments, and collaborating with medical staff for coordinated care.

This position is 90% remote with some onsite work required across Florida.
