Job description
Cardiac Care Alliance is hiring an experienced Registered Nurse Care Manager for a remote, hourly contract role focused on high-acuity cardiac patients. You will perform phone-based clinical assessments, develop care plans, and manage escalations while coordinating with cardiology teams and RPM programs.
Ideal candidates have 2+ years in cardiac care or care management, an active RN license in Texas or a compact state, and strong documentation skills.
