Job description
Seamlessassist is seeking a full-time, remote Clinical Pharmacist with strong cardiology experience to optimize GDMT, titration, and adherence for high-risk cardiac patients within a value-based care model.
The role involves collaboration with RN Care Managers and cardiology providers, is non-dispensing, and requires multi-state licensure with active Texas licensure. CT business hours alignment is expected.
