Job description
Seamless Assist is seeking a fully remote Clinical Pharmacist focused on Cardiology and Value-Based Care. This is a full-time, contract 40 hours/week role, primarily telephonic with nurses and cardiology providers, optimizing GDMT and titration while supporting adherence and outcomes for high-risk cardiac patients.
The pharmacist will not dispense medications but will integrate with the care team, document in EMR, and contribute to quality measures under CT business hours.
