Job description
Seamless Assist is seeking a remote Clinical Pharmacist with strong cardiology experience to optimize GDMT, support titration, and improve patient adherence for high-risk cardiac patients.
The role integrates with nurses and cardiology providers in a value-based care model and is not a dispensing position. Requires multi-state licensure and Texas license, with CT business hours alignment.
