Job description
seamlessassist is seeking a remote Clinical Pharmacist with a strong cardiology background to optimize GDMT, titration, and adherence for high-risk cardiac patients. The role integrates with nurses and cardiology providers in a value-based care model, with focus on clinical partnership rather than dispensing.
The clinical pharmacist will review medications, support titration, educate patients telephonically, and document interactions in the care management system to improve outcomes.
