Seamless Assist is seeking a remote Clinical Pharmacist with strong cardiology experience to support value-based care programs. This full-time, contract role focuses on GDMT optimization, medication titration, and patient education, collaborating with nurses and cardiology providers.
The position reviews med profiles, identifies gaps, and makes recommendations within protocols. Requires multi-state licensure and CT overlap; operates as a non-dispensing clinical partner in a nurse-led care model.
