Job description
UnitedHealth Group is seeking a Care Manager, RN to provide leadership in coordinating patient-centered care across the continuum. This role develops safe discharge plans through collaboration with patients/caregivers and multidisciplinary teams to arrange post-discharge services and optimize transitions in care.
The position allows telecommuting from anywhere in the U.S. and emphasizes improving LOS, patient experience, and reimbursement while reducing readmission risk.
