Job description
CVS Health Corporation in Illinois seeks a Case Management Registered Nurse to support Medicare/Medicaid members with complex health and social needs. You will collaborate with members, providers, and community organizations to improve care outcomes.
You will facilitate scheduling, assist with accessing benefits and education, and work from home with up to 75% travel across Metro East/Southern Illinois. A valid Illinois RN license and 3–5 years of clinical experience are required.
