Job description
Sentara Health Plans is hiring an Integrated Nurse Case Manager/Registered Nurse for Chronic/Complex Conditions. This is a full-time, work-from-home position that requires travel for face-to-face home visits in Norfolk, VA and surrounding areas.
The nurse will develop and monitor care plans, perform assessments, coordinate services, and liaise with Medical Directors and care teams to improve patient outcomes.
