Job description

CenterWell Primary Care seeks a Clinical Pharmacist to improve patient outcomes through medication management and education within a remote care team. You will support a full physician panel across locations, collaborate with physicians and nurses, and participate in multidisciplinary rounds to optimize therapy and reduce costs.

You will stay current with guidelines, perform post-discharge reconciliations, and contribute to CMS Stars and HEDIS initiatives.
