Upper Peninsula Health Plan (UPHP) seeks a Remote Community Based Clinical Coordinator - Care Manager to assess, plan, and coordinate care for members in the Luce County area. This full-time remote role includes home and facility visits as needed, coordinating services with providers and community-based resources.
