Job description
Zocalo Health is seeking an experienced operations leader to own the community health worker workforce and Community Supports within a growing, RN-led care model. You will guide deployment, performance reporting, and audit readiness across twelve counties and multiple health plans.
You will drive integration of community and clinical care teams, build scalable workflows, and design career paths for frontline staff, while ensuring compliance and sustainable unit economics across markets.
