Job description
Form Health, a virtual obesity medicine clinic, seeks an ABIM‑certified Endocrinologist with obesity medicine expertise for a fully remote US-based role. The physician will deliver evidence‑based obesity and diabetes care, collaborate with APPs and RDs, and guide complex cases including CKD/ESRD and CAD/CV conditions.
You will manage CGMs and insulin pumps, participate in telemedicine teams, and contribute to ongoing quality improvements while mentoring colleagues and trainees.
