Job description
Form Health, a virtual obesity medicine clinic, seeks an ABIM-boarded Endocrinologist to join a remote, part-time care team delivering diabetes and obesity management via telemedicine. A valid US medical license is required, with eligibility to obtain additional licenses as needed.
The physician will collaborate with Endocrine APPs and RDs, manage complex comorbidities with CGMs/insulin pumps, and participate in quality initiatives. 20 hours/week minimum, US-based.
