Job description
Form Health, a leading virtual obesity medicine clinic, seeks an ABIM-certified Endocrinologist with obesity medicine expertise to join a remote, physician-led care team. The physician will deliver evidence-based obesity and diabetes management via telemedicine, collaborating with APPs and RDs, and overseeing complex comorbidities using CGMs and insulin pumps.
The role requires an MD/DO, active US license, and ABIM certification, with ABOM preferred.
