Job description

Humana Inc. seeks a Clinical Pharmacist to join Conviva Senior Primary Care in a remote, physician-facing role. You will influence patient care through medication management, education, and collaboration with the care team.

You will support the full physician panel with various clinic locations, reporting to the Associate Director Integrated Clinic Pharmacist and integrating into the care team within the practice.
