Job description
Conviva Senior Primary Care, part of Humana, seeks a Clinical Pharmacist to improve patient outcomes through medication management and patient education within a remote primary care setting. You will support the full physician panel and collaborate with the care team across clinics to optimize pharmacotherapy.
